Failure Mode and Effects Analysis (FMEA) is a model that focuses on identifying the potential failure modes and their effects within a system or process. The analysis is used to prioritize potential failure modes according to the severity, number of possible causes, and number of potential detectors. The higher the priority, the more attention should be given to the mode and its effects. There are many types of an FMEA model, but the most commonly used is the Risk Priority Number (RPN). The RPN model uses a ranking system from 1 to 10, with 10 being the highest priority.
The FMEA model is used for both proactive and reactive risk management. In the proactive stage, the model is used to identify potential problems before they occur and develop solutions that eliminate or reduce the cause of the failure. This can be done by analyzing the system, process, and tasks involved to identify potential failure modes and analyze the effects they could have. In the reactive stage, organizations use the model to understand how existing failures or processes can be improved.
The FMEA model enables organizations to uncover potential risks and improve processes by making necessary changes in a timely manner before larger issues occur. This also enables organizations to understand the severity of a particular failure mode and its effect on an organization’s operations.

There are four main steps in the FMEA process: identify potential failure modes, determine their effects, prioritize the failure modes according to their risk levels, and develop corrective actions.
The first step of the FMEA model is to identify potential failure modes. This involves analyzing the system, process, and tasks that are involved in order to uncover potential failure modes. Once potential failure modes are identified, the organization can determine their effects which include the severity of the potential consequence, the likelihood of detecting it, and the probability of it happening. Organizations can then rank the failures from most to least severe according to their Risk Priority Number (RPN), which ranges from 1-10.
Once the failure modes are ranked, organizations can use the FMEA model to develop corrective actions. This involves determining which preventive and corrective actions need to be taken in order to reduce the risk level of the particular failure mode. This includes establishing procedures, policies, and training. Bed-time reviews are also commonly used to further utilize the FMEA model. The review should include a discussion about the current system, tasks, and activities as well as any potential errors or risks.
